All coaches and team managers will need the following clearances:


1. PA State Police Criminal Record Check Online:https://epatch.pa.gov/home(Good for 5
years)
a. Click “New Volunteer Record Check
b. Check box under Volunteer Ackonwledgement Section then click “Accept”
c. Fill out the form and click “Next” and click “Proceed” on the next page
i. Volunteer Organization name is Bellevue Avalon Athletic Association
d. Fill out the form on the next page with your personal information and click
“Submit This Request”
e. Email results to [email protected]

2. PA Child Abuse History Clearance Online:
https://www.compass.state.pa.us/cwis/public/home (Good for 5 years)
a. If this is your first time, click “Create Individual Account” then click “Next” on the
next page. If you have done this in the past, go to step 5.
b. Fill out the form to create your account then click “Finish”
i. Choose your own Keystone ID
c. If you have done this before, click “Individual Login”
i. Click “Access My Clearances” then click “Continue” at the bottom of the
next page
ii. Click “Create Clearance Application” then click “Begin” at the bottom of
the next page.
iii. Click "Volunteer Having Contact with Children…” for the
Application Purpose.
iv. Click other for Volunteer Category and type Bellevue Avalon
Athletic Association for Agency name
v. Click “Next at the bottom of the page
vi. Finish the rest of the application with your personal information
then click “Submit Application”

vii. There is no authorization code for the application when you get to
the last page of the form
